Building a Community Information Exchange

Government health and human service organizations are creating innovative Community Information Exchanges to improve the health and well-being of their communities by establishing cross-sector collaboration among human service agencies, social service providers, managed care plans, behavioral health providers, substance use disorder providers, and clinical providers. The goal is to enable organizations in different sectors using different technologies to work together to remove barriers to critical resources, identify gaps in care, promote transparency, and improve care coordination.
